Description : The large white mass seen behind the anterior inferior half of this tympanic membrane is a large congenital cholesteatoma.

The presence of any discrete white structure deep to the tympanic membrane should alert the examiner to the possibility of a congenital middle ear cholesteatoma.

Specialist referral is mandatory.
